Skylights and Floor Openings: The Fall Hazard Small Contractors Underestimate

OSHA treats skylights and floor openings as fall hazards, not features. Learn 1926.501 and 1910.23 requirements and practical protection options for small crews.

Ask a roofing or renovation crew to name their biggest fall hazard, and most will say the roof edge. They'll set up guardrails or a personal fall arrest system along the perimeter, feel good about the setup, and get to work. Then someone steps backward while carrying a bundle of shingles, or a laborer walks a straight line across a warehouse roof deck without looking down, and goes straight through a skylight that was never covered or marked. Skylights and floor openings are quietly one of the most consistent causes of fatal falls in small commercial construction and building maintenance, precisely because they don't look dangerous. A skylight looks like a window. A floor opening in a mezzanine looks like a gap someone will obviously notice. Neither assumption holds up under OSHA's numbers, and neither holds up under the standard.

What the Standard Actually Requires

Under 29 CFR 1926.501(b)(4), workers on a walking-working surface with a hole must be protected from falling through it, regardless of the hole's depth, by a personal fall arrest system, a covers, or a guardrail system erected around the opening. Critically, the rule doesn't distinguish between a small hole and a large one, or a hole a worker might step in versus one they might fall through entirely — any hole gets protected. For skylights specifically, OSHA has been explicit for decades: a skylight is treated as a hole in the walking-working surface unless it's guarded or covered to the standard's specifications, even though it looks like part of the roof. This shows up constantly in citations after roof-related fatalities, where the skylight had no cover, no screen, and no guardrail, and the employer's defense was that everyone knew where the skylights were. Familiarity is not a control measure under 1926.501, and OSHA's Directorate of Construction has said so directly in interpretation letters going back to the 1990s.

For general industry work — warehouses, manufacturing floors, and other non-construction settings — the parallel requirement lives in 1910.23(a)(4), which covers floor holes, and 1910.28 for fall protection generally. The logic is the same: any floor hole into which a person can accidentally walk, work near, or fall must be guarded by a cover or a guardrail system on all exposed sides, or the worker must use a personal fall arrest system.

Covers Have to Meet a Real Standard, Not Just Look Like One

A sheet of plywood laid over a hole is not automatically compliant, and this is where a lot of well-intentioned small contractors get tripped up. Under 1926.502(i), covers for holes must be capable of supporting twice the maximum intended load of persons, equipment, and materials that may be imposed on the cover at any one time, and they must be secured to prevent accidental displacement by wind, equipment, or workers. They also need to be color-coded or marked with the word "HOLE" or "COVER" so nobody mistakes the cover for solid decking during a later phase of work. A piece of OSB screwed down loosely at two corners, with no load rating and no marking, will not hold up as a defense if it fails or if an inspector walks the site. If you're covering a skylight opening during roofing work, that cover needs to span the full opening and rest on framing members capable of bearing the load, not just balance across the skylight curb.

The Sequencing Problem on Renovation and Reroofing Jobs

Floor and roof openings change day to day on active jobs, which is exactly why they get missed. A hole that was covered and marked on Monday might get uncovered Tuesday for HVAC rough-in, then left open overnight because the crew assumed someone else would re-cover it before the next shift. This is where a daily walk of the work area, specifically looking for anything that opened, moved, or lost its cover since the last shift, pays for itself. Assign it to whoever is running point on-site each morning, and make it a two-minute habit rather than a paperwork exercise. On reroofing jobs in particular, mark every skylight location on a rough site sketch before tear-off begins, so the location is documented independent of whether the skylight itself is still visually obvious once old roofing material is staged nearby.

Training Workers to See the Hazard, Not Just Avoid the Edge

Most fall protection training for small crews understandably focuses on harness use and anchor points, because that's the gear conversation. But a worker wearing a properly rigged harness can still fall through an unmarked skylight if they're not clipped to an anchor at that moment, which is common during material staging or when moving between tasks. Toolbox talks should specifically call out that skylights and floor holes are treated as unprotected edges under the fall protection plan, not incidental features of the work surface, and that any uncovered or unguarded opening gets reported and barricaded immediately, not worked around. A five-minute conversation before a reroofing or demolition job, walking the crew past every known opening, does more to prevent a fatality than another repetition of general harness instructions.

Small contractors who build hole and skylight protection into their daily routine, rather than treating it as a one-time setup at the start of a job, are the ones who avoid both the citation and the far worse outcome it's meant to prevent.
